A large observational study has uncovered an unexpected connection between the type of shingles vaccine a person receives and their long-term cardiovascular risk. The research, which tracked over 72,000 matched adults aged 60 and older in the United States, found that those who received the newer recombinant vaccine had a 9 percent lower overall burden of cardiovascular disease over a seven-year period compared to those who got the older live attenuated vaccine.

This natural experiment emerged from a real-world shift in vaccine availability. As the US transitioned from the live vaccine to the recombinant version, researchers were able to compare health outcomes between two similar groups. The benefits were not uniform across all conditions. Both men and women showed lower rates of ischemic heart disease and heart failure. However, the reduction in ischemic stroke was significant only among men.

The findings suggest that the recombinant vaccine may offer protective effects beyond its primary purpose of preventing shingles. While the exact biological mechanism remains unclear, researchers believe the immune response triggered by the recombinant vaccine might influence vascular health. The study's authors emphasize that these results are observational and do not prove cause and effect. They are calling for randomized clinical trials and further mechanistic studies to confirm the pattern and understand why the vaccine might influence heart and brain circulation. If validated, this could add another layer of consideration for vaccine recommendations in older populations.
