Seven Health Issues Set to Shape the 2026 Midterms

A doctor's guide to the health questions voters say matter most heading into the 2026 midterms, from rising costs and insurance premiums to Medicaid, fraud and abortion.

Health care is again moving toward the center of the national debate as candidates build their campaigns around the cost of care and access to treatment. A physician-authored guide highlights seven issues that voters consistently rank as priorities, offering a plain look at what is at stake and where the political fault lines sit.

At the top of the list is the price of care. Families continue to report difficulty absorbing premiums, deductibles and out-of-pocket bills, and candidates in both parties are under pressure to explain how they would ease that burden. Closely tied to this is the cost of insurance sold through the Affordable Care Act marketplaces, where premium increases and the future of federal subsidies have become a recurring flashpoint.

Medicaid also features prominently. Proposals to adjust federal funding, tighten eligibility rules or expand coverage have drawn sharp disagreement, with rural hospitals and nursing homes warning that cuts could destabilize local care. Abortion remains one of the most divisive health questions on the ballot, with state-level rules and national proposals continuing to drive turnout on both sides.

The guide also examines fraud and waste in federal health programs, an issue that draws rare bipartisan attention as lawmakers look for savings. Prescription drug prices and the implementation of Medicare negotiation provisions round out the list, alongside concerns about the health care workforce, including shortages of nurses and primary care doctors in underserved areas.

The overall picture is one of voters who are skeptical of broad promises and focused on concrete costs. Candidates who can speak clearly about premiums, access and local hospital stability are likely to have an advantage, while vague talking points on health care may struggle to land.
